The road to greater mobility is a journey we will travel together. From our initial contact through meeting your mobility goals, this is how we get started:

Step 1:
Our initial phone call

To determine whether my professional experience might be a good fit based on your particular needs, we will set up an initial phone call to:

  • Gain a better understanding of your unique situation

  • Discuss ways I can help with your physical therapy needs

  • Examine private pay options

  • Answer any questions

  • Explore the availability of

  • additional community resources

Step 2:
An in-home consultation

We will set up an in-home visit to learn more about which goals are most important to you.

 

We will gain understanding of your prior mobility level as well as assess your muscle strength, range of motion, balance and gait in order to identify your physical capabilities, limitations, and specific needs.

Step 3:
A written assessment

We will provide a personal physical therapy plan showing:

​

  • realistic goals to improve and/or restore your overall functional mobility and fitness level.

  • personally designed exercises to complete during our visits

  • self-guided exercises to help you continue to improve between visits

Our programs are personally designed to maximize safety and independence, postpone the onset and/or minimize progression of illness, and enhance the quality of living in your own home.
